Sound familiar?

How hip bursitis usually shows up.

If a few of these ring true, you are in the right place. None of them mean something is badly wrong, and all of them respond to the right plan.
  • A sore point on the outside of the hip you can press on
  • Lying on that side in bed wakes you
  • Stairs, hills and getting up from a chair pinch
  • Standing on one leg to put on pants is hard
  • Sitting with legs crossed makes it worse
  • You have had an injection that helped for a few weeks then wore off

Why it happens

What is usually going on.

What used to be called trochanteric bursitis is now understood as gluteal tendinopathy, or greater trochanteric pain syndrome. The gluteal tendons on the side of the hip get compressed and overloaded, and the bursa is sometimes irritated with them. It is most common in women over 40 and in runners and walkers who have changed their load.
Because the problem is a tendon that has lost tolerance to load, the fix is to change how the hip is loaded day to day and to progressively strengthen the gluteal muscles. Positions that compress the tendon, like crossing your legs or hanging on one hip, are the early things to change.

What we do

Assess, plan, retest. Numbers, not guesswork.

Every plan at Integral Movement starts with measurement on our force plates and strength testing, ends with a written program, and is retested at six weeks. One clinician, start to finish.

01

Assess

Single leg control, gluteal strength and endurance, gait, and the specific tests that separate tendon pain from hip joint or back referred pain.

02

Unload and load

Practical changes to sleeping, sitting and standing to take the compression off, plus a graded gluteal strengthening program that starts within tolerance.

03

Progress

Load increases every couple of weeks toward stairs, hills, walking distance and running. Retest at six weeks.

What the research says

Evidence, in plain English.

We read the research so you do not have to. These are the findings that shape how we treat hip bursitis. Ask us for the papers at your visit.

01

The LEAP trial (BMJ, 2018) found that education plus exercise for gluteal tendinopathy beat corticosteroid injection at 8 weeks and at one year, and beat ‘wait and see’ as well.

02

Tendon research consistently shows that progressive loading, not rest, restores a tendon’s capacity.

03

Injections can give short term relief but do not change the tendon and are associated with poorer results at 12 months compared with exercise.

FAQ

Questions about hip bursitis

Usually both are part of the same picture, and the tendon is the main driver. The treatment is the same: reduce compression, then load the glutes progressively.

Stretching the ITB or glutes often makes this condition worse because it compresses the tendon. We will show you what to do instead.

Yes, with some changes to distance, hills and footwear early on. Walking is part of the plan, not something to stop.

Tendons change slowly. Most people feel meaningfully better in eight to twelve weeks and continue improving over months.
